27 April 2015
Indiana MENTOR is pleased to welcome staff and individuals served from Snug Harbor Home Health, a provider of supported living services based in Daleville, Ind. The addition expands Indiana MENTOR’s supported living services for adults with intellectual and developmental disabilities in the Hoosier state.
“Snug Harbor Home Health has a long history of providing individualized residential supports that respect the choice and life goals of each person they serve, making them an ideal addition to Indiana MENTOR,” said Dan Mohnke, Executive Director of Indiana MENTOR. “We are excited by the opportunity to provide these life-enhancing services to more individuals in their homes, where they can grow and increase their independence.”
Snug Harbor Home Health has provided high-quality supported living services to adults with intellectual and developmental disabilities, including those with complex needs such as behavioral challenges for more than XX years. Staff provide support in an individual’s home on an hourly basis as well as 24-hour support, depending on the individual’s needs and goals. Service offerings include supports such as help with meals and personal hygiene, transportation and wellness coordination.
“As we grow, we become stronger as an organization, with more to offer the people we serve. The addition of Snug Harbor Home Health allows us to strengthen our footprint in the Daleville and Anderson areas of Eastern Indiana. We are excited to begin serving more individuals in these communities,” Mohnke said.
Snug Harbor Home Health is now operating as Indiana MENTOR.
